Kindergarten registration at T. Clay Wood April 9 and April 23

T. Clay Wood Elementary School at 10600 Kettle Run Road, Nokesville, VA 20181, will hold kindergarten registration for the 2015-16 school year Thurs., April 9,  from 10 a.m. to 5 p.m. and Thurs., April 23, from noon to 7 p.m.

Students who accompany their parents will receive a T.Clay Wood T shirt.

Documentation needed for registration includes:
• Original Birth Certificate (Child must be 5 years old by Sept. 30, 2015)
• Social Security Card - Original
• Proof of Residency – 3 items
• Physical Examination – Physical MUST be performed within one year prior to school entrance (8/31/14 or after). Physical must be signed, stamped and dated by your doctor.*

• Immunizations Record:
DTaP - A minimum of 4 doses with one given after 4th birthday
Polio – A minimum of 4 doses with one given after 4th birthday
MMRs – 2 Measles, 2 Mumps, 1 Rubella - one after 1st birthday and 2nd dose prior to entering kindergarten
HEPB – 3 doses given over a 6 month period (Dose 2: 4-8 weeks after dose 1. Dose 3: 8 weeks after dose 2. Total of 16 weeks between dose 1 & 3.)
Varicella (Chicken Pox Vaccine) – 2 doses after first birthday

*Physical forms are not needed at registration; however, they are required before the first day of school 8/31/15.
